Well, it is still legal in this country to pay directly for services performed directly to the provider, especially in elective procedures, such as plastic surgery or lasik surgery etc. Notice how in these fields the flow chart looks like this:
Both charts that Philip has posted reflect way too much government for my taste, I would rather go with the chart I just typed for the entire health care system, perhaps introducing the element of voluntary and unregulated insurance (real insurance - i.e. premiums paid not for daily use of services but for protection against rare and unpredictable events only).
